Stabilization Program for the Homeless
MVA’s Stabilization Program was developed as a form of outreach to aid the homeless population in Marion and Monongahelia counties. Our stabilization social worker is proactive in the identification and assessment of individuals experiencing homelessness and acts as a referral source for the relevant community resources they need.
Services Include:
- Access to MVA’s primary care, optical and pharmacy services
- Access to MVA’s behavioral health and case management services
- Appointment scheduling
- Medicaid outreach counselor referrals
- Housing referrals
- SOAR applications
- Community outreach and education
- Crisis referrals
- Street outreach
- Linkage and referral to community safety net services
- VI-Spidat/HMIS entry
- Point-In-Time Count Lead Agency
