MVA is looking for a contracted LICSW/LPC that completes intake sessions. Develops individual care plans for behavioral health patients. Provides clinical evaluation, intervention, and planning for patients to improve patients/families functioning, overall patient wellness, and medical adherence. Provides risk assessment and intervention, with emphasis on reducing harm. Facilitates crisis intervention and emergencies services when necessary. Provides recommendations and coordination of external psychiatric and/or social resources based on assessment and collaboration with the patient/family. Acts as liaison between patient/family, behavioral health providers and medical health providers to ensure clear communication amongst the care team . Provides documents and reports for referrals. Ensures adequate documentation of patient encounters for billing purposes and to ensure a continuum of care. Provides case management around psychosocial/behavioral health needs. Identifies patients who are at high risk and/or have multiple or complex psychosocial needs.
Provide clinical assessment of patients to overcome obstacles in self-management of chronic disease and health behavior. Must be a licensed social worker. MVA is an EEO.

MVA Health Centers Inc. is currently looking for a full-time Case Manager. This candidate will be responsible for working closely with the LICSW to complete annual tracking, scheduling, and authorizations for behavioral health patients. Also, this candidate will be providing supportive counseling and case management to patients. The candidate will assist patients with healthcare insurance coverage and provide education and assistance to the community. Bachelor’s degree in social work, psychology, nursing, or another health or human services field is required. Experience is preferred. MVA is an EEO employer.
Front Office Staff
MVA Fairmont Clinic has an opening for a full-time Front Office position. The successful candidate must have a strong customer service background, computer skills, be able to work in a busy environment and be very dependable. Experience with health care insurances a plus.
