Executive / Administrative Assistant
Monongahela Valley Association of Health Centers, Inc., a Federally Qualified Health Center, is seeking a full-time Administrative / Executive Assistant. Primary Care/Family Medicine Physician
MVA Health Centers is seeking a Primary Care/Family Medicine Physician for Fairmont, WV. MVA is a Federally Qualified Health Center that offers many services such as primary care, pediatrics, internal medicine, an on-site pharmacy, radiology, optical services, behavioral health, school-based health, and Express Care; Monday through Friday. Outpatient services to be provided include acute, chronic, preventative, and other direct patient care visits. Referral to specialty and follow-up as needed. Provider will be responsible for shared call a week at a time. Full-time; competitive salary, benefits, and leave package; including malpractice insurance coverage. EOE.
- Must have M.D., D.O., or foreign equivalent degree
- Be BE/BC in Family Medicine;
- Primary care experience and Board Certification preferred
This candidate will complete intake sessions. Develop individual care plans for behavioral health patients. Provide clinical evaluation, intervention, and planning for patients to improve patients/families functioning, overall patient wellness, and medical adherence. Provide risk assessment and intervention, with an emphasis on reducing harm. Facilitates crisis intervention and emergencies services when necessary. Provide recommendations and coordination of external psychiatric and/or social resources based on assessment and collaboration with the patient/family. Acts as liaison between patient/family, behavioral health providers and medical health providers to ensure clear communication amongst the care team . Provides documents and reports for referrals. Ensures adequate documentation of patient encounters for billing purposes and to ensure a continuum of care. Provides case management around psychosocial/behavioral health needs. Identifies patients who are at high risk and/or have multiple or complex psychosocial needs.
Provide clinical assessment of patients to overcome obstacles in self-management of chronic disease and health behavior. Must be a licensed social worker or LPC. MVA is an EEO.
